As part of the Beyond the C project, there are two key focus areas for reporting to the team at ASHM. These include the completion of the:
Timings:
Data search results – Please share your data search results at the end of Step 2 and Step 4 of the project. This will report on the numbers and demographics found in the initial search, and those tested and treated. It is important to include the data from all people tested and treated for hepatitis C during the project period.
PDSA templates – It is recommended that you share your completed PDSA templates at the end of Step 3 and Step 4, however, you are welcome to discuss these plans with your Practice Nurse Advisor at any point in the project.
Data search results
When you report your data search results to the Beyond the C project team, you help to contribute to the goal of finding patients living with untreated hepatitis C. This data will be aggregated to help us understand the demographics of patients who may not yet have received treatment.
You will need to use the Data search spreadsheet to capture and collate data about the patient information you find in your search. It also includes functionality to automatically aggregate results for sharing with ASHM. Your Practice Nurse Advisor will provide more guidance on how to use this tool.
The data you share will be aggregated with de-identified information from all other project participants, providing a big-picture view of the impact of the Beyond the C project.
PDSA templates
Your completed PDSA templates track your practice’s progress in the project, including the strategies you implemented to reach patients, and how effective they have been. After the project, ASHM will share results on the most effective strategies for finding and treating people with hepatitis C.
Step 3: Clinical audit provides guidance and tools to support you to complete these PDSA templates.
When you have completed your templates, and you have reflected on the results of your activities, you can share them with us by emailing the ASHM team.
