About the program
The Beyond the C program is part of a national program called ‘Finding 50,000’.
Steps in the journey
In this program, you will conduct a clinical audit, using your medical records system to help you identify people who could be tested for hepatitis C. You will also follow quality improvement processes to follow up these people and help connect them with ongoing care.
There are four steps in the process.
Project roles
Your team will need to nominate people to fulfil the following roles (team members within your practice may take on one or more of these roles):
Champion
- Leads the project
- Arranges hepatitis C and stigma education for the practice
- Completes project reporting
Data searcher
- Completes the hepatitis C data searches
- Applies filters as required
- Develops the audit list
Clinical auditor
- Reviews each file on the audit list
- Applies the standardised coding for hepatitis C to each file
- Develops the non-stigmatising recall strategy and recall list
Clinician
- Arranges hepatitis C testing
- Refers people who have complex issues
- Prescribes and monitors DAA treatment
Support for the journey
Each role in the project needs different training and support. Throughout the project, you will have access to:
- A comprehensive package of support that provides guidance for each step, including checklists and tools
- Online learning modules on hepatitis C including case-finding, testing, treatment and support
- Practice resources that you and your practice staff can use to support people accessing care.
You will also have the support of your PHN or HHS/LHD, as well as personalised support from a Practice Nurse Advisor who will provide support to your practice over the phone/video call and via emails.
Time commitment
The project must be completed within 6 months. The time commitment for your practice will depend on several factors including your practice’s data management and experience with quality improvement projects – you can finish the project earlier if all learning needs and reporting commitments have been completed.

This project offers your practice an opportunity to develop processes and skills in clinical auditing and quality improvement – all with the personalised support of an expert, along with access to high-quality training and communication resources. The skills and processes you learn here will be invaluable in improving your quality of care in all areas of patient care.
In addition, the program helps you to align with the Quality Improvement Practice Incentive Program, and attracts an additional Beyond the C program payment of $1,500. This additional payment is delivered in two 50% payments when you complete certain project milestones.
