Introduction

The aim of this questionnaire is to collect feedback on your practice’s experience of the Beyond the C Project.

You have been invited because you are the nominated key point of contact within your practice for this project. If you feel there is a more appropriate person in your practice, they may complete the survey on your behalf.

Your participation is voluntary. The questionnaire will take approximately 10-15 minutes to complete.

The results of the evaluation will be used for internal purposes, quality improvement of the project for any future iterations and for required funding reports. Any information provided in funding reports will be de-identified. For example, we won't link a comment with a person's role or where they work.
