Process of identifying
The Beyond the C project will guide you through a process of identifying people in your patient records who may have hepatitis C, and connecting with them to offer testing and treatment.
A key part of the project is the continuous improvement process – as you complete each step, you’ll be encouraged to plan your actions, implement them, and reflect on how they worked. And then – keep going! It’s an iterative process that will help you refine your methods of finding, reaching and helping your patients.
There are four stages in the project. Each step has different tasks and timeframes.
